Kill the Clipboard: Standardized Intake Package
Patients enter habits, supplements and diet once, and get a standard package any clinic can read.
What it does
- A 5-step patient wizard that checks each step before moving on.
- Exports a FHIR R4 Bundle (validated against the official HL7 FHIR R4 schema in testing), a one-page printable summary and a QR code.
- A provider view reads the package back from a file, pasted text, a photo of the QR code or the camera, runs validation checks and shows a readable summary with screening prompts.
- Saved intakes are listed in the browser, with open and delete.
Where it runs and what stays private
Runs entirely in the browser: no server and no account. Drafts stay in this browser.
Good to know
- Today it is a demo with fictional personas, and it is not for clinical use.
- Real clinical use would need a privacy and security review and an EHR integration.
- Tested in Chromium-based browsers. The QR code needs Firefox 113+ or Safari 16.4+.
